Application to Enroll in POAM Extended Legal Representation Plan

I acknowledge that I have reviewed the terms and conditions of the POAM Extended Criminal Legal Representation Plan as described at poam.net. By applying for enrollment in the Plan I agree that coverage under the Plan will become effective upon POAM's approval of my application for enrollment and receipt of the membership payment.

Payment Notice: Upon POAM's acceptance of your application for enrollment, a billing invoice will be transmitted for payment of the annual $60.00 membership fee, which will be prorated for the balance of the current Plan year. The Plan year begins May 1st of each year, ending on April 30th of the following year.
